For most of its modern history, the peer review system has operated on a straightforward moral logic: a reviewer who fears professional reprisal will soften legitimate criticism, so anonymity is the price science pays for candor. The arrangement has served well enough that it became the default architecture of academic publishing worldwide. But a mounting body of documented cases — and a growing chorus of researchers willing to name what they have witnessed — suggests that the same veil protecting honest reviewers is also shielding a far less admirable cast of characters.

The problem is not hypothetical. It is structural.

A System Designed for Trust, Exploited for Advantage

Double-blind peer review, in which neither author nor reviewer knows the other's identity, remains the dominant model at major journals across the life sciences, social sciences, and engineering disciplines. The rationale is sound in principle: eliminate the halo effect that prestigious institutional affiliations confer, protect early-career researchers from senior gatekeepers with grudges, and allow reviewers to deliver unflattering verdicts without damaging collegial relationships.

What that architecture also does, however, is eliminate accountability almost entirely. A reviewer who delays a competitor's manuscript for months while racing to publish similar findings faces no formal consequence. A reviewer who systematically recommends rejection for work that contradicts their own published positions leaves no traceable fingerprint. A reviewer who requests the addition of self-serving citations — a practice documented in multiple journal investigations — does so from behind an institutional curtain that most journals lack the resources or mandate to lift.

A 2017 analysis published in Research Policy estimated that citation manipulation through peer review may affect as many as one in five manuscripts in certain fields. More recent surveys of researchers at US institutions have found that a substantial minority — ranging from roughly 12 to 20 percent depending on career stage and discipline — report having personally witnessed or experienced what they characterized as retaliatory or strategically obstructive reviewing. These are not fringe complaints. They are systemic signals.

High-Profile Failures and What They Revealed

Several cases in recent years have illustrated with unusual clarity what happens when reviewer anonymity intersects with competitive pressure and inadequate editorial oversight.

In 2020, a controversy surrounding a clinical nutrition study at a major US research university drew attention when internal communications leaked during a misconduct investigation suggested that a reviewer had requested specific methodological changes that would have, incidentally, undermined a finding that directly contradicted the reviewer's own prior work. The journal involved declined to identify the reviewer, citing confidentiality obligations. The study's authors were left with no formal avenue for appeal.

Similarly, ongoing scrutiny of several high-profile retractions in the social psychology literature has revealed patterns in which manuscripts were delayed through multiple revision cycles — sometimes spanning two or more years — before being rejected on grounds that critics argue were available at the initial submission stage. In cases where reviewers' identities were later disclosed through voluntary transparency initiatives, conflicts of interest were identified in a disproportionate share of those extended-delay rejections.

None of this proves coordinated bad faith at scale. But it establishes that the current system provides insufficient friction against it.

The Accountability Gap and Emerging Responses

The tension here is genuinely difficult. Abolishing reviewer anonymity carries real costs. Research consistently demonstrates that open review — in which reviewer identities are disclosed, either to authors or publicly — reduces reviewer participation rates, particularly among junior scholars who reasonably fear senior colleagues' reactions to critical assessments. A 2021 study in PLOS ONE found that open review systems generated shorter, less substantive reviewer comments on average, suggesting that the threat of visibility does measurably constrain candor.

The challenge, then, is not to dismantle anonymity but to architect accountability around it.

Several emerging models are attempting exactly that. Post-publication review platforms such as PREreview and the overlay journal model pioneered by organizations like eLife have introduced structured transparency at the editorial level without necessarily exposing individual reviewer identities to authors. Under these frameworks, journals retain reviewer identity records that are accessible to independent ethics boards — a kind of sealed-envelope accountability that preserves confidentiality while creating a credible deterrent against abuse.

Other institutions are experimenting with reviewer credentialing systems, in which reviewers are verified and their review histories — stripped of manuscript-identifying details — are compiled into longitudinal records. Patterns of systematic delay, disproportionate rejection rates for specific research methodologies, or unusually frequent citation requests can then be flagged algorithmically without requiring full disclosure. Several preprint servers operating in the biomedical space have piloted early versions of this approach with promising initial results.

The US National Institutes of Health has, in recent years, signaled increased interest in reviewer accountability as part of broader research integrity initiatives, though formal policy has not yet followed that interest into binding guidance.

Calibrating Reform Without Collapsing the System

Any meaningful reform must grapple honestly with the fact that peer review, for all its documented flaws, remains the most scalable quality-filtering mechanism science has produced. Replacing it wholesale is neither feasible nor obviously desirable. The more tractable goal is closing the specific accountability gaps that allow misconduct to operate invisibly.

At minimum, journals should be required to maintain auditable reviewer records accessible to institutional ethics bodies upon complaint. Conflict-of-interest disclosure requirements — already standard for authors at most reputable outlets — should be extended with equal force to reviewers, with verification mechanisms rather than honor-system declarations. Editorial boards should receive training in identifying behavioral signatures of strategic obstruction, and authors should have access to formal appeal channels that do not require them to identify a specific wrongdoer.

The peer review system was built on the assumption that most participants would act in good faith. That assumption is probably still correct. But a system's integrity cannot be indexed to the behavior of its most ethical participants — it must be robust against its least ethical ones. Science's credibility depends on getting that architecture right.
